suffers from one of the following:

  • Headaches
  • Memory loss
  • Movement disorder
  • Neuro-muscular disease
  • Seizures
  • Stroke
  • Unexplained peripheral neuropathy
  • Deterioration in vision
  • Unexplained deterioration in results of neuro-psych testing
  • Excessive daytime sleepiness
  • Unexplained fatigue
  • Snoring
